The Care Advisor conducts intake and provides proactive customer service for individuals who may be eligible for Seniorlink services. This includes inbound and outbound communication with prospective care recipients, caregivers, and professional referral sources. This position works as part of a centralized intake and support team to qualify and enable enrollment into Seniorlink programs across multiple states. Serving as the organization’s “front door”, this individual shares information about Seniorlink services, gathers eligibility information and routes individuals to the appropriate stop along the enrollment process. This involves gathering and documenting eligibility information including demographic, clinical and financial qualification criteria. The Care Advisor will provide guidance to caregivers, ensuring excellent customer service and advocacy. The ideal candidate will need to work full time including on Sundays.

In collaboration with state and MCO case managers, the Remote Care Manager provides dedicated support, education, and guidance to caregivers, giving them tools and confidence to enable better care for loved ones at home. Support and resources are provided via telephony and proprietary digital chat technology, using evidence-based methods of care that address both health care and social needs. The Remote Care Manager typically handles more complex, higher risk/need cases on the Coaching Ops team, which may span across multiple states and/or Seniorlink programs.
