Please note this person will be going to Weymouth, Whitman,Dedham, Dorchester, Roxbury, West Roxbury, Roslindale,Mattapan,Hyde Park,Jamaica Plain, South, Boston, South End.The Care Manager works collaboratively with the care management team to assess consumers, establish and implement a person-centric plan of care, conduct regularly scheduled homes visits, and oversee consumer care. The care management team credentials caregivers and their homes and provides ongoing support and training.
Reporting to the Director, Care Management, the Manager, Care Management is responsible for directing care management operations and leading a team to deliver services that engage, motivate, and educate family caregivers to support complex individuals, keeping care at home. This individual ensures the delivery of high-quality services in compliance with all state, program, and accreditation requirements. This individual will manage a team of clinical and non-clinical care managers, providing case management services across the continuum of care (assessment, care planning, care coordination, and monitoring and evaluating services). In collaboration with key stakeholders, this individual will create action plans in support of quality improvement efforts and positive health outcomes.

The Assessment, CM coaches, advocates and educates caregivers to assist with the daily well-being needs of consumers. This position supports the ongoing care management needs of cases as needed.
