Use your RN skills to travel to families where you get to assess and qualify families who need our support. This is a non-clinical role that is solution based and requires no nights or weekends.
The Assessment, RN Care Manager (RN) works collaboratively as part of a care team to guide prospective families through an assessment process to enable efficient activation of services. This includes assessing consumer and caregiver eligibility for services, developing an initial person centric care plan and preparing families for ongoing services including orientation to a digital communication platform, Vela. Assessments are carried out within established timelines for expedient service activation. This person will travel to Boston, Braintree, Quincy, and surrounding areas.
