  • Position Summary:

    The Social Work Care Manager works collaboratively with the care management team to assess, establish and implement a person centered plan of care. To assist families obtain needed financial benefits and community resources. Counsel to help families cope with long term illness.

    What You Will Do:

     

    • Identifies community resources, determines financial eligibility and assists with applications for Medicaid, waiver programs, and other state benefits as needed.
    • Maintains knowledge of changes to federal and state programs. Required to track changes to CT Medicaid, and 5 waiver programs including income and asset changes, number of slots allowed within waiver programs and length of current waitlist. Communicates with CH government relations department to ensure current knowledge of regulatory or benefit changes.
    • Works with MFP program director, care managers and transition coordinators to assist consumers discharging from long term SNF placement to integrate into the community setting. Attend MFP steering committee meetings to maintain knowledge of program changes.
    • Responsible to educate CGH CT managers, sales team, field and admin staff regarding any program/eligibility changes in CT.
    • Collaborates and communicates regularly with CGH clinical team, access agency care managers, support services, primary care physician, and other medical providers to ensure timely and appropriate delivery of care.

    What You Will Bring:

    • Master’s degree in social work required. Minimum two (2) years of experience in community based healthcare including eldercare, people with disabilities and other complex medical needs. Successful experience working with and collaborating nurses in a regulated healthcare setting. Valid driver’s license.
