  • Position Summary:

    The Community Engagement Specialist works locally in collaboration with remote coaches and other professionals to support family caregivers who are caring for a loved one at home. The Community Engagement Specialist visits families at home and conducts assessments of a consumer’s medical, behavioral, and social needs and barriers to care and identifies the support a caregiver needs to help provide care to the consumer. This individual will partner with a centralized remote coach team to ensure ongoing engagement, support, and intervention as needed. Position concentrates in the Sioux Falls/SE area of the state.

    What You Will Do:

    • Conduct initial and annual assessments of caregivers and consumers in their homes; conduct periodic interim face-to-face visits if necessary; input all assessment material and notes into case management system
    • Help caregivers to compile required caregiver credentialing documents and ensure the caregiver is able to access Seniorlink’s technology for communication between caregivers and coaches.
    • Work with the Leadership to develop thoughtful and strategic approaches to reach families in culturally diverse communities.
    • Build collaborative partnerships with local community organizations and Tribal communities to promote the benefits of Structured Family Caregiving and help identify consumers and families that need support.
    • Represent Seniorlink at provider, advocacy and partnership meetings as necessary.
    • Significant travel within the state of South Dakota

    What You Will Bring:

    • Bachelor’s degree in social work, or an equivalent combination of education and experience in related fields
    • Minimum two (2) years of experience in case management and program operations in the areas of healthcare and social services, including but not limited to working with elders, people with disabilities and other complex medical needs in a healthcare, long-term care, social service or community setting
    • Proficient and comfortable working in a case management system and with applications on mobile devices
    • Must have a valid driver's license and a reliable, insured vehicle
