The Clinical Team Lead (CTL) is the lead escalation point for clinical issue resolution and management of emerging clinical issues. The CTL works with care teams to manage risk, track and respond to changes, and identify the need for at-risk case management processes. The CTL works with care teams to ensure compliance with documentation standards, required regulations, and best practices. The CTL carries a half-time Care Manager caseload.
Bachelor’s degree in a health care or social services discipline required or equivalent Registered Nurse experience. Minimum of three (3) years of clinical experience in case management and program operations in the areas of healthcare and social services, including but not limited to eldercare, people with disabilities and other complex medical needs. Additional state-specific qualifications may be required. Valid driver's license required.
